Our office is open Monday through Thursday from 8:00 a.m. until noon and 1:00 until 5:00 p.m., Friday from 8:00 a.m. until noon.  We are closed for holidays.

Our office hours are by appointment only. If you must cancel an appointment, please do so as much in advance as possible.

If a problem arises before your next scheduled appointment, please call our office and check with the doctor or nurse about being seen sooner.

After office hours or on days when the office is closed, you can reach a physician by calling our office at (765) 286-0236. Your call will be answered by the physicians' answering service. You will be referred to the physician on call if your physician is not available.

Fees

Our fees are within the customary range for this area and reflect the level of care you will receive as well as the high level of training by the physicians here.

We have standardized charges for various procedures. These fees are listed in the office and are available to you at all times.

You may receive more than one bill for services rendered here. You will receive one for the professional services of your doctor, any supplies used and most laboratory tests drawn here. Any procedures done outside our office will be billed from the appropriate facility. If you have any questions about fees or need to make financial arrangements, please feel free to discuss them with our billing department.

At your first visit, all pertinent insurance information will be taken. We bill most Insurance carriers directly. Unless otherwise indicated, you are responsible for your portion of the bill subject to deductibles and co-insurance percentages. You are responsible for any remaining balance not covered by insurance. If you have financial difficulties, please let our billing department know immediately. We will help if we can.

Medical Records and Confidentiality

All your medical records are confidential. We will release the records or any parts of them to individuals or agencies as requested by you. No records will be released to anyone without your written authorization. No matter the circumstances, your records cannot be released without your written consent.
